Doctors note?

i went into hospial 2 weeks ago (wisdom teeth extraction) and needed the whole week off uni, the lady in the office said i probably wouldn't need a doctors note but now my clinical lecturer said i do need one. how should i go about this? ring my dentist or the hospital? will i be charged for one?
